An unwell hen: what to record for the vet
A concise timeline is often more useful than a long list of guessed diseases. Start a record as soon as you notice a concern, and keep product packaging if anything has already been given.
Record the basics
Identify the bird or pen, approximate age, current weight if safely obtainable, first abnormal sign and the time it began. Note feed, water, laying, droppings, breathing and whether other birds are affected. Add recent arrivals, diet changes and heat exposure.
Record every treatment
Write the exact product, concentration, route, amount, time and who advised it. Do not report only the brand if several formulations exist. Include anything put in communal water or feed, even if intended for one hen.
Keep observations useful
Take a clear photograph or short video if it does not delay urgent care. Follow the vet's advice about samples and transport. Do not cut, squeeze or repeatedly manipulate a bird just to obtain more information.
Next step: use the health log and keep a copy of the treatment instructions. Your kit can hold the paperwork as well as supplies, so important details are available when you call.
